Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).
PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

HOME CARE COORDINATOR RN
(HH RN FIELD SUPERVISOR)
Under the supervision of the Home Health Director, the RN supervisor will be primarily
responsible for the Personal Assistance Service Attendant component of the home health
department. The RN will directly supervise the Home Care Coordinator LVN and PAS
Scheduler.
RESPONSIBILITIES:
1. In the absence of the Home Health Administrator RN, the Home Care Coordinator RN will take on all of the Home Health Administrator RN’s responsibilities by abiding by the Home Health Administrator RN’s job description.
2. Performs home health assessments for initial in-home service referrals within ten working days of participant’s enrollment. Develops the appropriate individualized home health service plan and related paperwork.
3. Completes routine reassessments and care plan by participant’s I&A date.
4. Communicates participant’s changes to the IDT at the I&A meetings or the daily participant care planning meeting.
5. Completes participant’s reassessments and Care Plan due to changes in participant’s condition expeditiously as required by their condition.
6. Completes reassessments at the request of participant/designated representative within 72 hours of request.
7. Coordinates with participant/family, caregiver and contracted providers to ensure 24 hours service delivery.
8. Assures that participants are assigned/provided the appropriate personnel, including self, when the care requires a higher level of skills.
9. Documents in participant’s medical records and HH chart as required by departmental policy and procedure.
10. Assists with the recruitment, credentialing and selection for employment according to PAS component and BIENVIVIR personal policies.
11. Performs on-call duties as per departmental supervisor.
12. Provides guidance to LVN Field Supervisors on dealing with problems or potential problems between PAS attendants and participant/family (i.e. time utilization, PAS duties, etc.)
13. Supervises and provides guidance to Scheduler in assigning cases to attendants based on level of care identified.
14. Develops and implements a training and evaluation program for inexperienced PAS attendants.
15. Submits required paperwork in a timely manner. Posts MSUR home visits within the next working day of service delivered.
16. Updates and maintains Quality Assurance Plan, performs QA activities and prepares monthly departmental QA reports.
17. Prepares and presents monthly mandatory in-services for PAS attendants. Coordinates in-services by scheduling outside speakers or with other B.S.H.S disciplines, as needed.
18. Provides on-the-job training to new employees during the initial orientation and continues thereafter as outlined by the BIENVIVIR Home Health departmental policies.
19. May delegate tasks to PAS attendants in accordance to Board of Nurse Examiners and Texas Department of Health memorandum.
20. Assists with evaluation visits to participants receiving personal care services as part of scheduled I&A. Records visits according to BIENVIVIR policy.
21. Performs written evaluation on PAS attendants at the end of the six-months probationary period and yearly thereafter.
22. Participates in selected committees as needed in the development of the organization’s policies and procedures. Participates in monthly supervisory and QA meetings.
23. Participates in IDT conferences and/or specific discipline/family conferences.
24. Reports weekly or as needed to Home Health Director giving summary of the week’s work.
25. Reports all problem areas in non-skilled component to the Home Health Director and conducts problem solving.
26. Other assignments as delegated by Home Health Director.
27. Responsible for hospital/NH visits for Home Health discharge planning to assess for PAS Attendant services and/or avoid interruption in continuity of care upon discharge.
28. Evaluates and verifies hours of services rendered to participants during weekly review of attendant timesheets.
QUALIFICATIONS / REQUIREMENTS:
1. Registered nurse with current state registration (B.S.N preferred).
2. Preferred experience in Home Health Services or supervisory capacity.
3. Past clinical experience in a hospital setting preferred.
4. TB test or current CXR.
5. Current CPR certification.
6. Bilingual capability (Spanish/English) preferred.
7. Possess valid driver’s license and auto insurance.
8. Maintain personal professional requirements for licensure.
9. Comply with mandatory monthly in-service hours
Required Skills
Required Experience
A High School Diploma or equivalent is preferred but not required. Qualified candidates will have 3-5 years of experience or a technical degree and 1 year of experience. Basic PLC/CNC knowledge is a plus. The ability to read to read tape measures and efficiently operate all other required measuring devices is also required. Candidates best suited for this role will have a proven ability to work effectively independently and in a team environment. Basic ability to read, write, speak, and understand English is required for safety purposes.
