Responsibilities:
Responsible for handling all PHC/Attendant care documentation and files to meet state requirement.
Responsible for scheduling agency patient visits.
Provide staffing feedback to DON/Supervisor when needed.
Assess, plan, and coordinate changes in patient care services.
Responsible for the supplying needs of all clients of the agency.
Assigns and verifies that all nurses, aides and other disciplines are in compliance with documentation (timely submission and QA), and that the daily schedule is being followed.
Report any deficiencies to DON.
Maintains and updates patient information to enhance highest quality patient care.
Consults with all members of the treatment team and DON or regular basis to coordinate quality, goal oriented home health care.
Serves as liaison between patients, caregivers, doctors and doctors office staff, agency administrative staff and contracted services
Assists in the billing process and financial functions as needed.
Oversees Agency communications including pagers, telephones, mail and tracking of physician orders.
Promotes compliance with all state and federal regulations.
Organizes, sorts, and maintains documentation in patient files in a timely manner.
Provides clerical support which may include preparation of admission, orientation and new hire packets; typing, special projects, filing and coping.
Assists with maintenance of personnel files.
Assists with communications which may include: phone system, memo’s, answers incoming calls, documents and disseminates appropriate messages.
Provides computer support and data entry, as needed.
Promotes the agency philosophy and mission by presenting a positive image to patients/families, physicians and community agencies.
Provide effective communication to patient/family, team members, and other health care professionals and maintain confidentiality.
Receives and appropriately documents new referrals in a timely manner.
Participates in coordinating care with Nursing Supervisors, patients/clients, and the interdisciplinary team.
Provides effective communication to patients/clients, staff members, other health care professionals and referral sources.
Responds to Agency and patient/client needs in a professional and creative manner.
Demonstrates commitment, professional growth and competency.
Participates in Agency-sponsored in-service training.
Participates in the Agency’s QAPI program.
Promotes the Agency philosophy and administrative policies to ensure quality of care.
Performs other duties as required.
