    Qualifications:

    Graduate of an accredited, state licensed School of Nursing.

    Current, active, and unrestricted LPN license in the State of Georgia

    Minimum of 1-3 years nursing experience preferred

    Experience in adult day health care, geriatrics, dementia care, long-term care, or home health preferred

    Knowledge of chronic disease management and age-related conditions

    Strong documentation, communication, and organizational skills

    Ability to work independently and collaboratively within a multidisciplinary team

    Works well with seniors in providing nursing care.

    The Licensed Practical Nurse (LPN) at SarahCare at Riverdale Place Adult Day Health Care provides compassionate, participant-centered nursing care in a structured daytime setting. The LPN supports the health, safety, and well-being of participants by delivering nursing services, monitoring medical conditions, and collaborating with the interdisciplinary team to enhance quality of life while allowing participants to remain safely in their homes.

    Essential Duties and Responsibilities

    Conduct daily nursing assessments and monitor participants for changes in physical, emotional, or cognitive status.

    Administer medications and treatments in accordance with physician orders, RN supervision, and SarahCare policies.

    Dispose of expired/discontinued medications appropriately.

    Monitor vital signs, blood glucose levels, and other clinical indicators as required.

    Accurately document nursing care, monthly progress notes, incident reports, medical questionnaires, medication orders, and changes in condition using WellSky Electronic Health Record (EHR) as required.

    Observe and report signs of illness, injury, aspiration, or behavioral changes promptly.

    Provide first aid and respond to medical emergencies in accordance with established protocols.

    Assist with development, implementation, and review of participant care plans.

    Communicate effectively with families, caregivers, VA staff, social workers, and healthcare providers.

    Support participants with activities of daily living (ADLs) as needed, including toileting, mobility and mealtime assistance, and in the participation of activities as needed.

    Ensure compliance with SarahCare standards, VA requirements, HIPAA, OSHA, and infection control protocols.

    Participate in inspections, audits, and quality assurance activities.

    Participate in Participant Intake Assessment and Person-Centered Plan of Care conferences.

    Participate in staff meetings, in-service training, group discussions, and other relevant training.

    Promote a safe, respectful, and welcoming environment for participants, families, and staff.

    Request supplies and items needed for nursing through the Program Director.

    Provide skilled nursing duties under RN supervision.

    Ability to safely lift and transfer.
