About the Role
We are currently seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide in-home assessments, care coordination, and clinical support for Medicare-age patients throughout Wake County, North Carolina. This is an opportunity to work directly with seniors in their homes, helping improve health outcomes, reduce avoidable hospitalizations, and support patients with complex medical and social needs.
Responsibilities
- Conduct comprehensive in-home health assessments for Medicare Advantage members
- Perform medication reviews, health screenings, and risk assessments
- Identify medical, behavioral, and social factors that may impact patient outcomes
- Educate patients and caregivers on disease management, medications, and preventive care
- Develop and support individualized care plans based on patient needs
- Coordinate care with primary care providers, specialists, and ancillary healthcare partners
- Collaborate with physicians, nurses, social workers, care coordinators, and health coaches
- Identify care gaps and recommend interventions to improve quality outcomes
- Document patient encounters and care recommendations accurately within the electronic medical record
- Support initiatives focused on reducing hospitalizations, improving chronic disease management, and enhancing patient quality of life
Qualifications
- Master's Degree from an accredited Nurse Practitioner or Physician Assistant program
- Active North Carolina Nurse Practitioner or Physician Assistant license
- Active board certification as a Nurse Practitioner or Physician Assistant
- Active NPI Number
- Active DEA Registration
- Current BLS Certification
- Valid driver's license and reliable transportation with current automobile insurance
- Strong communication and relationship-building skills
- Excellent organizational, problem-solving, and time management abilities
- Ability to work independently in a fast-paced field-based environment
Preferred Skills
- One or more years of clinical experience as an NP or PA
- Home health, home-based primary care, or community-based care experience
- Experience caring for geriatric or Medicare populations
- Chronic disease management experience
- Previous electronic medical record (EMR) experience
- Experience working within value-based care or managed care environments
Pay range and compensation package
Salary Range: $130,332 - $195,498 annually. Actual compensation will be based on experience, education, licensure, certifications, geographic location, and overall qualifications.
