Job description
We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA-C) to join our team at a Rehabilitation Center in Concord NH. This role offers a unique 7 days on / 7 days off schedule, providing an excellent work-life balance. The ideal candidate will have 1-2 years of experience as an Advanced Practice Provider (APP) and a passion for providing high-quality rehabilitative and post-acute care.
Key Responsibilities:
- Conduct comprehensive patient assessments, develop treatment plans, and provide ongoing medical management.
- Collaborate with an interdisciplinary team, including physicians, therapists, and nursing staff, to ensure optimal patient outcomes.
- Monitor patient progress, adjust care plans as needed, and provide patient and family education.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures within scope of practice.
- Ensure compliance with all regulatory and facility guidelines.
Qualifications & Requirements:
- 1-2 years of experience as a Nurse Practitioner or Physician Assistant (required).
- Current NP or PA license in NH.
- Experience in rehabilitation, post-acute, or skilled nursing facility (SNF) settings is preferred.
- Strong clinical skills, critical thinking, and ability to work independently.
- Excellent communication and collaboration skills.
