 Provide comprehensive, integrative care across multiple settings — in-office and via telehealth  Support women through perimenopause, menopause, and related hormonal transitions by integrating HRT management into preventive and lifestyle-based care.  Evaluate, diagnose, and treat patients following evidence-based care pathways, including management of chronic conditions, preventive care, and hormone optimization.  Collaborate with physicians and specialists on complex cases to ensure holistic treatment plans.  Guide patients through their wellness journey, educating them on lifestyle, nutrition, hormone balance, and preventive health strategies.  Communicate proactively and compassionately with patients to deliver an exceptional experience.  Conduct health risk assessments, manage ongoing treatment plans, and ensure continuity of care.  Leverage technology and integrated EMR systems to streamline workflows and maintain documentation accuracy.

 3+ years of clinical experience, preferably in primary care, endocrinology, GYN, or restorative health  Demonstrated experience in hormone therapy (bioidentical hormone replacement, anti-aging medicine, functional medicine)  Master’s degree from an accredited NP or PA program  Active, unrestricted Georgia NP or PA license  Active or eligible DEA license in Georgia  Ability to work in the Woodstock, GA office

Qualifications  Proven ability in conducting patient assessments, diagnosis, and treatment planning.  Strong knowledge of patient education practices and preventive care strategies.

 Experience in hormone therapy (bioidentical hormone replacement, anti- aging medicine, functional medicine)  Proficiency in maintaining accurate medical records and understanding of compliance with healthcare regulations.  Master’s degree from an accredited NP or PA program.  Active, unrestricted Georgia NP or PA license.  Active or eligible DEA license in Georgia.  Effective interpersonal and communication skills to engage with diverse patient populations and teams.  Experience in a clinical or outpatient care setting preferred.
