    Evolutionary Healthcare is seeking compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our innovative Osteopathic Medicine team serving skilled nursing and post-acute care facilities throughout Central Florida.

    This unique role is ideal for providers who enjoy hands-on patient care, musculoskeletal medicine, pain management, and working closely with Osteopathic Physicians to improve mobility, function, and quality of life. While OMT is performed by our physicians, you will play a vital role in identifying appropriate patients, managing their medical care, and supporting comprehensive treatment plans.

    What You'll Do

    As an OMT Support Provider, you will:

    Provide comprehensive primary care services in skilled nursing and post-acute care facilities.

    Evaluate patients experiencing musculoskeletal pain, mobility limitations, balance disorders, and functional decline.

    Identify patients who may benefit from Osteopathic Manipulative Treatment and collaborate with our OMT physicians on treatment planning.

    Manage acute and chronic medical conditions that affect rehabilitation and recovery.

    Perform admission evaluations, routine follow-up visits, and post-hospital assessments.

    Coordinate care with physicians, therapists, nursing staff, and facility leadership.

    Monitor patient progress and help optimize functional outcomes.

    Educate patients and families regarding treatment plans and preventive care.

    Maintain timely, accurate documentation using electronic health records.

    Ideal Candidate

    Qualifications

    Licensed Nurse Practitioner or Physician Assistant in Florida (or ability to obtain)

    National Board Certification

    DEA license (or ability to obtain)

    Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, or Post-Acute Care preferred

    Skilled Nursing experience is a plus but not required

    Strong communication and clinical decision-making skills

    Passion for patient-centered care
