    Support Coordinator 2+ Years - Experienced Support Coordinator Needed - Part-time Position in Roselle, NJ, US We are seeking an experienced Support Coordinator for a part-time role in Roselle, NJ. As a Support Coordinator, you will be responsible for analyzing service requirements, devising service plans, and linking clients with the necessary programs and providers. You will also need to continuously observe and provide advocacy, consultation, and instruction to clients and their families. Additionally, you will perform monthly monitoring tools and make visits to clients as required. Key Responsibilities: - Analyze service requirements and create service plans - Connect clients with appropriate programs and service providers - Continuously observe and provide support to clients and their families - Perform monthly monitoring tools - Conduct regular visits to clients Key Job Functions: - Assess and complete a Person-Centered Planning Tool [PCPT] with clients and their families - Use NJCAT to identify assistance needs - Coordinate and facilitate meetings to determine service needs and design individual support plans - Assist in finding services and supports to achieve desired outcomes - Construct and update service plans - Support individuals and families in ensuring chosen services meet desired outcomes - Input all data into the electronic record system - Provide quality case management and maintain regular contact with clients and their support team - Attend meetings and professional training as required Compensation: - $120+ per case, per month (10-20 cases) Benefits: - Growth prospects - Flexible work schedule - Salary increases Responsibilities: • Support Coordination is managed by a specific person (the Support Coordinator) assigned to each participant • The Support Coordinator is liable for producing and maintaining the Individualized Service Plan together with the participant, their family, and other participants chosen by the participant • Support Coordinators are accountable for providing assistance with coordination for persons with intellectual and developmental disabilities • The Support Coordinator is responsible for constantly evaluating the provision of services integrated into the Individualized Service Plan Qualifications: • Excellent oral and written communication and presentation skills • Professional demeanor and appearance • Excellent public speaker • Bachelor's degree in any field • Excellent client relations and customer service skills • Excellent organization and time management skills • Must have a valid driver’s license and personal vehicle-driving to client homes, and other locations are required • Must have a computer or laptop • Completion of ALL New Jersey DDD/CDS required training or willingness to complete training • COVID-19 Vaccinated • This position requires travel • Must have 1+ years working with adults with disabilities • Must have home office Compensation: $120+ per case per month
