Position Overview
The Clinical Director is responsible for the overall direction of hospice clinical services.
The Clinical Director establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
Essential Job Functions

Must have reliable transportation to travel to patient homes per patient schedule and agency needs
Using effective body mechanics, must be able to lift 40 lbs. floor to waist; lift 10 lbs. waist to shoulder; lift and carry 40 lbs. and push/pull 40 lbs.
Must be able to knowledgeably and competently deliver nursing care or physical therapy to patients
Must be able to (or assist with) lift, turn, move and position patients
Must be able to ensure clinically competent/qualified staffing is appropriate to meet the needs of the patients served
Must be able to competently direct and oversee patient care, ensuring care is clinically appropriate
Must be able to ensure efficient, appropriate, and cost effective clinical operations of the agency
Must be able to modify/approve clinical paperwork, as well as perform chart audits and provide associate education when necessary

Must have excellent verbal, written, and interpersonal communication skills
Must have valid driver’s license and auto liability insurance
Must possess ability and willingness to work harmoniously with professional and non-professional personnel
Must possess the ability to make independent decisions when circumstances warrant such action
Must understand and follow company policies and procedures
Must have knowledge of governmental regulations and JCAHO standards (if applicable)
Must be able to multi task and accept additional duties as assigned
Must present a professional image
Must be knowledgeable of nursing or physical therapy practices and procedures based on state practice acts
Must be competent in standard Microsoft Office applications, internet, and email
Must have knowledge of hospice Medicare, state regulations, governmental regulations and JCAHO standards (if applicable)
Must possess leadership and supervisory skills
Education and Experience
Registered Nurse or Physical Therapist with current licensure to practice professional nursing or therapy in the state where agency located
Bachelor’s degree in Nursing from an accredited program by the National League for Nursing or Bachelor’s degree in Physical Therapy. Master’s degree in health care preferred
Three years of management experience in a hospice or related health care organization
Demonstrated ability to supervise and direct professional administrative personnel
Ability to market and deal tactfully with customers and the community
Has excellent observation, verbal and written communication skills
Knowledge of business management, governmental regulations and ACHC standards
Has been oriented to the Executive Director/Administrator job responsibilities
Working Conditions
Works in office environment
Sits, stands, bends, lifts, and moves intermittently during working hours
Subject to frequent interruptions
May work beyond normal working hours
May work in the field doing patient visits
Attends and participates in continuing education programs
Involved with patients, associates, caregivers, government agencies/personnel, etc. under various circumstances
General Responsibilities
Coordinates and oversees all direct and indirect patient services provided by the clinical organization personnel
Provides guidance and counseling to the clinical staff to assist them in continually improving all aspects of hospice care services, provided through organization personnel
Assists Clinical Supervisors in managing clinical teams and planning
Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated
Assists the CEO in the preparation and administration of the organization's budget
Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses
Evaluates performance of Clinical Staff
Assists Clinical Staff to develop skills, techniques and knowledge that make them compliant experts in end of life care. With exceptional customer service skills
Assists CEO in Hiring, evaluating, and terminating clinical personnel
Conducts clinical performance evaluations a minimum of semi-annually, or more frequently if indicated
Oversees and is responsible for the content and compliance of every clinical note, order, practice and process produced or performed by the clinical staff.
Assures proper maintenance and timely submission of all clinical records and documents from clinical staff to medical records in compliance with local, state and federal laws and organizational policy.
Responsible for the proper training and use of supplies and equipment for the provision of patient services
Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel.
Participates in the planning, development and implementation of in-services and continuing education programs to meet education and training needs of organization personnel
Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization
Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures
Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the ACHC Hospice Care standards
Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel
Promotes hospice referrals in the health care community
Responsible for the day to day clinical operations of agency
Ensures appropriate staffing levels to meet patient needs. Submits requests to CEO for additional staff as needed
Responsible for holding clinical staff accountable for meeting all productivity and quality standards.
Evaluates the clinical competency and performance of nursing staff. Performs joint visits with associates at least semi-annually and documents their performance
Coordinates the on call schedule, weekend schedule, and vacation schedule for all clinical staff.
Ensures patient care is performed according to agency policy and regulatory/state requirements
Responds to patient complaints in a timely and thorough manner with customer service as top priority. Documents complaints and communicates with associates in response to complaints as appropriate. Communicates with patient regarding outcome of complaint
Assists in auditing of charts in response to Adverse Event and OBQI reports. Utilizes audit results in educating associates regarding clinical care of the patient. Assists in auditing 20% recertification audits and discharge audits
Participates in regulatory agency reviews
Responsible for new associate orientation coordination
Accessible for and resource for associates needing clinical guidance
Utilizes software system reports in clinical management of patients and staff
Accountable to counsel and develop associates in the meeting of their job expectations
Works cooperatively with intake and marketing staff in accepting new referrals
Review clinical notes of staff to ensure appropriate documentation. Educates staff on best practices related to documentation
Maintains confidentiality of all patient and associate information
Encourages associate participation in Performance Improvement program including documenting on occurrence reports, and participation in OBQI work groups
Performs visits and admissions as needed based on staffing availability and coverage during vacations or illness
Collects/trends all required data for PI program utilizing appropriate forms. Submits PI quarterly results to Compliance and Quality Manager
In the absence of the CEO, the PCC will become the acting CEO and will be vested with authority to act in behalf of the CEO
Other duties as assigned by the Executive Director/Administrator
