Job Overview:
As the Agency Nurse, you will play a vital role in providing comprehensive medical care and support to individuals residing in a residential care setting. You will work collaboratively with a multidisciplinary team to ensure the health, safety, and well-being of the residents. Your responsibilities will include administering medical treatments, managing health records, implementing care plans, and maintaining a supportive environment for the individuals under your care.
Responsibilities:
- Provide prescribed treatment and healthcare procedures as directed by the physician.
- Creating and distributing Individual Plan Medical/Health Reports, outlining participants' overall health, healthcare goals, and physician's recommendations for each participant.
- Document and share the individual's health status with relevant support people, such as family members and/or caregivers.
- Promptly inform supervisor of any concerns or issues related to individuals' health needs.
- Advocate for individuals by seeking medical care based on needs assessment.
- Make appropriate referrals for necessary medical services or consultations.
- Conduct quarterly and as needed in-service training for support staff and managers for knowledge and skills enhancement.
- Ensure MARs are accurate and documented appropriately, through monthly reviews and unannounced audits.
- Schedule and coordinate medical appointments while working with team to ensure all medical appointments are completed and paperwork is accurately filled per 6400 regulations.
- Maintain an appointment tracker per agency protocol; ensure all medical paperwork are saved electronically and paper copy filed in program book.
- Assist in creating a strong support network, connecting the individual, family members, and the community.
- Review all medical paperwork and ensure follow-up appointments are scheduled and completed.
- Ensure all doctors' orders are carried out promptly.
- Attend specialized medical appointment as needed and determined by the team.
- Ensure that individuals' insurances are up to date with the appropriate coverage.
- Manage the inventory of medical supplies (company and individuals') and place orders when supplies are running low.
- Rate individuals as a HRST Rater
- Collaborate with Managers to address medication errors or other medical incidents.
- Investigate incidents as assigned.
- Be available for on-call duties as required, to address urgent medical needs.
- Will be required to travel between sites
- And other job duties as assigned.

Qualifications:
- Valid RN/LPN License in the state of PA (or valid Compact State RN/LPN)
- HRST experience preferred, not required.
- Complete all state and agency required training per state guidelines.
- Previous experience working with people having intellectual disabilities preferred.
- Three years of experience working with Residential Community homes.
- One year of clinical experience or other appropriate experience.
- Strong interpersonal and communication skills with the ability to work as a team.
- Experience with problem solving against multiple priorities.
- Knowledge and understanding of regulatory agency operations (6400).
- Competent Computer Skills, including MS Office
- Excellent time management skills.
- Must demonstrate strong attention to detail and be well organized.
- State of PA Criminal Background check
- FBI clearance required for individuals who do not meet PA residency requirement
Working Conditions:
- Location- CLA Sites, office.
- Valid driver's license with a satisfactory driving record
- May be required to lift and/or physically support consumers independently or with other staff.
