Job Description

• Participates as an active member of the interdisciplinary team, determining the nutrition and dietary goals of participants' care plans and monitoring progress toward those goals
• Completes accurate, timely assessments of participants' nutritional status
• Monitors monthly weight, taking into account factors such as medical diagnosis and drug-nutrient interactions
• Develops nutrition-related treatment plans in conjunction with the interdisciplinary team
• Provides nutrition counseling to participants

Qualifications

• Bachelor's Degree with major studies in food and nutrition or dietetics with ADA registration and PA licensure (LDN)
• Two (2) to five (5) years experience preferred
• At least one year of experience working with older adults
