    AleraCare, a provider of infusion treatments and specialty pharmacy services, has an immediate need for a PRN Infusion Registered Nurse for our San Jose, CA. and Mountain View, CA. infusion centers. The centers are open Monday – Friday, 8:00am-5:00pm. We have over 35 ambulatory infusion centers across 6 western states and we’re continuing to grow with our services.     Objective

    The Infusion Registered Nurse is responsible for providing nursing services to assigned patients in accordance with the patient’s plan of care.

    Essential Duties

    • Responsible for the clinical record for each patient receiving nursing care.
    • Assures that progress reports are made to the physician for patients receiving nursing services, when the patient’s condition changes, or there are deviations from the plan of care.
    • Adheres to facility’s policies and procedures, including patient care policies.
    • Performs initial patient evaluations / assessments and develops a written plan of care.
    • Regularly re-assesses the patient’s condition and nursing needs and up-dates the plan of care.
    • Furnishes those services requiring substantial and specialized nursing skill.
    • Initiates appropriate preventive nursing procedures.
    • Prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient's condition and needs.
    • Counsels the patient and family in meeting nursing and related needs.
    • Demonstrates competency in providing patient care and provides care in accordance with professional standards and state’s nurse practice act.
    • Initiates case conferences and communicates patient plan of care with team members.
    • Evaluates and re-evaluates the need for services.
    • Provides appropriate patient education and teaching materials.
    • Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
    • Participate in committees when requested.
    • Attends continuing education programs and staff meetings as appropriate to job responsibilities.
    • Represents facility in a positive and professional manner.
    • Perform other duties as assigned by supervisor.

    Minimum Qualifications

    • Graduate of an accredited program and current RN license.
    • At least one year of nursing experience, infusion nursing experience is preferred.
    • Effective interpersonal, time management and organizational skills.
    • Strong communication skills in speaking and writing.
    • Organizational skills sufficient to maintain consistently accurate records.
    • Ability to recognize and direct information to the appropriate health care provider.
    • Computer skills that include word processing, and efficient use of the internet and e-mail.
    • Current CPR card, health certificate, valid driver’s license, reliable transportation, automobile insurance.
