    WHAT WE’RE LOOKING FOR:

    We are seeking to hire a self-motivated, efficient, responsible Certified Home Health Aide for a large established company within the Medical–Skilled Nursing Industry with many growth opportunities. This is a direct hire: Full Time or Part Time permanent positions are available. If you have a passion for healthcare, this is the right opportunity for you!

    As a member of the interdisciplinary team, the Certified Home Health Aide provides personal care services to patients and performs related tasks in the patient/family place of residence in accordance with the plan of treatment as assigned by the RN Case Manager.

    WHAT YOU’LL DO:

    • Duties assigned are to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient and to provide for temporary respite and relief from total care responsibilities for the primary caregiver
    • Perform other duties as assigned
    • Monitor patient condition by observing physical and mental condition, intake and output, and exercise
    • Support patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands
    • Assist patients by providing personal services, such as, bathing, dressing, and grooming
    • Help patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene
    • Help family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient; advising on nutrition, cleanliness, and housekeeping
    • Record patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions
    • Maintain a safe, secure, and healthy patient environment by following asepses standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards
    • Adhere to professional standards, home care policies and procedures, federal, state, and local requirements

    WHAT YOU’LL BRING:

    • Previous experience as CHHA
    • Current CHHA certification in the state of CA required and CPR certification preferred
    • Demonstrates excellent observation, problem solving, verbal and written communication
    • Ability to work with minimal supervision
