    Large well known managed care company has openings for either RN or LPN with chronic condition medication experience This is a long term temp position

    Description:

    Position involved in functions such as pharmacy medication adherence management, patient telephonic outreach,, quality management, etc. which require advanced medical training i.e. RN, LPN, etc. This is a combination telecommuting/on-site (Regional office) position

    Contractors on this assignment will be working remote and will be expected to work in office 1-2 days per week,

    All candidates must be flexible regarding in office work, have reliable transportation and a valid driver’s license . Staff will work on Part D pharmacy 5 STAR measures and will manage medication adherence, refills due, 100 day conversion opportunities, communicate with patients, provider offices, pharmacies, etc. Home internet is required.

    Candidates must have telephonic, Excel, patient education and chronic condition medication knowledge. Seeking an RN or LVN

    In this position, you will work with the Nurse and pharmacy team to take our organization to 5 STAR performance.

    The purposes of this role include, but are not limited to:
    1. Health Plan specific outreach efforts and provider outreach
    2. Medication adherence, refills due, conversion opportunities, mail order opportunities, additional duties as assigned.
    3. Health plan meetings

    Responsibilities:
    • Receives HP reports and organizes priorities to manage Part D measures: including medication adherence, refills due, conversion opportunities, patient education, high volume calls

    • Integrates data collected from a variety of electronic and paper sources

    • Maintains HIPAA standards and confidentiality of protected health information; reports critical incidents immediately.

    • Attends conference calls as necessary to provide information relating to Part D measures and compliance

    • Performs other duties as assigned

    Required Qualifications:
    • RN/LVN license
    • Will consider a new grad.
    • Solid knowledge of managed care requirements related to pharmacy measures
    • Research and detail oriented, excellent proof reading skills
    • Excellent time management and prioritization skills
    • Strong communication skills
    • Strong proficiency with Microsoft Office applications, including: Word, Excel, Adobe and PowerPoint
    • Critical thinking skills
    • Results-driven independent worker, and self-starter
    • Experience working with HEDIS software (data entry, reporting) / Will consider a new grad.
    • Experience in medical record review (vendor or health plan; onsite or fax/mail)
    • Experience working in EMR systems

    Training: 2-3 week training on Medication adherence, refill opportunities, statins, Part D measures, HP meetings aw.

    Hours are Mon-Fri- 8-5
