RN Embedded Care Coordinator

Duties: The ECC plays a pivotal role in the P4Q program by providing onsite practice support to assigned participating Primary Care Providers (PCPs) groups and health systems. The ECC role can be summarized in three (3) key areas of responsibility: Partnership with PCPs / practice staff to improve member outcomes Member care coordination and gap closure Medical Record Review/Retrieval and data abstraction Skills: Excellent verbal and written communication and interpersonal skills, including primary care provider relations Excellent Microsoft Outlook, Word, Excel skills; Ability to access and utilize multiple systems and software Strong telephonic case management skills Ability to learn new processes and systems quickly Detailed oriented Strong time management and organization skills Ability to multitask and set priorities Strong research, analytical and problem solving skills Ability to work independently with minimal supervision Effective patient coaching/engagement and motivational interviewing skills Ability to work in rapidly changing environment Keywords: Education: Requirements: Current licensure as a Registered Nurse (RN), active and in good standing in applicable state Must have Alabama RN licensure.

Current BLS CPR certification Minimum of two years clinical practice experience coordinating care or doing telephonic case management for adults/seniors living in the community. Minimum of two years clinical practice experience in field based/primary care providers office setting Travel requirement 75%. Ability to drive own vehicle daily and work at participating practices.

Full-time standard hours are 9:00 AM-5:00 PM Mon-Fri, excluding company holidays
