A Special Project Nurse/Medical Professional will be responsible for completing auditing and/or medical records review projects. Responsibilities may include medical records reviews, data collection, chart abstractions, reporting and analyses. Must have good communication skills to interface with United Health Services personnel and be deadline oriented. Specific Duties:

1. Telephonic patient interaction
2. Use interactive voice recognition tool
3. Schedule patient appointments
4. Coordinate patient's care with various departments/services Helping the patients enroll in federal and/or state funded programs.
5. They will also help the patients resolve barriers with their access to care, like transportation issues, etc.
6. Document and report all patient outreaches
7. Work with member retention team to collect data Must have qualifications/experience:
8. 2 years clinical experience with older adults
9. Bilingual English\Spanish
10. Good verbal skills
11. Proficient in Excel and Word
12. Fluid computer skills
13. Outlook
14. Multi-line telephone system Need the role to be bilingual- English/Spanish.
