Description:

The 2 LVNs will be:
1. Making patient and provider phone calls
2. They will also help the patients resolve barriers with their access to care, like transportation issues, etc.
3. Document and report all patient and provider outreaches

The roles need to be proficient in the use of Outlook, multi-line telephone system, word and excel.

Need the role to be bilingual- English/Spanish.

Hours M-F 8am-5pm CST
Training: 8am-5pm CST


A Special Project Nurse/Medical Professional will be responsible for completing auditing and/or medical records review projects. Responsibilities may include medical records reviews, data collection, chart abstractions, reporting and analyses. Must have good communication skills to interface with United Health Services personnel and be deadline oriented. Willingness to travel a must, as this position involves working on site at various client locations as needed.
