Job Title: Inpatient coordinator

Assists the Clinical Services Inpatient Nurse with the facilitation of the inpatient admit process. This includes, but is not limited to, the collection of clinical data that is utilized for concurrent review, data entry and the facilitation of discharge planning such as follow up outreach, ordering of DME/Home Health and other post-discharge outpatient services. The Clinical services coordinator may not make decisions which require clinical judgment.

• Collects data for clinical review and submits data to licensed clinical services staff for decision-making
• Authorizes selected services per physician orders.
• Performs post-discharge follow up calls.
• Arranges post-discharge appointments.
• Performs data entry
• Prepares NOMNCs and collects information for DENCs.
• Files/faxes
• Other duties as assigned.

What software skills are needed? Must know how to type. Use of MS Office. Use of telephone, copier and fax machines

What are the hours? ( Training hours?) 8am – 5pm (1 hour lunch) or 8:30am – 5pm (30 min lunch).
