Licensed social worker for large managed care company - temp to hire position - must be fully vaccinated for COVID

Job description 1. Initial Assessments to identify discharge needs
2\. Care coordination related to discharge planning
3\. Meet with patients and families to discuss plan of care and patient choice
4\. Meet CMS compliance requirements for discharge planning and conditions of participation
5\. Document plan of care in electronic health record timely and thoroughly
6\. May consult with members for the multi-disciplinary team in liaison with community resources to support the patients treatment
7\. Assists patients/families with obtaining community assistance by referral to proper resources
8\. Develops and maintains working relationships with community health, welfare, and social agencies and seeks creative means to assist the patient with his/her needs
9\. Active participant with other members for the team regarding opportunities for improvement in standard work

education: BSW required, MSW preferred. Active WI License

software skills EPIC experience preferred
