    Title: 

    Complex Health Solutions Spiritual and Emotional Support Care Specialist

    **Long Term/Full Time/Contract Role for a large managed care facility.

    The Spiritual and Emotional Support Care Specialist will be responsible for providing direct guidance, counsel, and support to our members as well as to the nationwide team of Care and Research Advisors and clinicians within the Family Engagement Center. The primary focus of their work will be related to the emotional and spiritual well-being of our members and caregivers dealing with complex health conditions.


    Principal Duties and Responsibilities:


    • Participate in delivering and arranging for appropriate spiritual and emotional care to members, their loved ones, and staff.

    • Actively participate in the delivery of spiritual care adhering to the Standards of Practice of a nationally recognized certifying body and the principles and practices of Outcome Oriented Chaplaincy.

    • Respond to requests for spiritual care in a timely manner.

    • Provide short term care and guidance to members, their caregivers, and the team

    • Assess need for ongoing care and connect to appropriate resources within the member’s community if longer term support is necessary.

    • Pro-actively integrate into the operations/front line team, as well as product/support team, to provide longitudinal spiritual support based on relationship, rapport, and trust.

    • Participate in regular staff meetings, direct report meetings, and strategy meetings.

    • Participate in regular professional development activities and supervision.

    • Advocate for spiritual care/person-centeredness in each professional interaction.

    • Maintain accurate records of member- and staff-facing spiritual care activity to consistently communicate with appropriate members of the Complex Health Solutions, and, where appropriate, with Enterprise-level chaplaincy efforts.

    • Participate in the Complex Health Solutions’ planning and strategy for continuous quality improvement.

    • Coordinate with the Complex Health Solutions team, Care Specialists and among the other chaplains throughout the Enterprise to ensure consistent communication and process.

    • Create didactic content as Subject Matter Expert in chaplaincy-related arenas, such as: diversity & inclusion, communication, spirituality, end of life, grief, compassion, etc.

    • Participate in team meetings and rounds.

    • Ability to conduct a thorough Spiritual Assessment and identify areas of strength/support
      Ability to facilitate a reconnection to internal and external sources of meaning

    • Assist members with clarification of personal values and desires of clinical care

    • Make appropriate referrals to additional resources as appropriate (e.g. support group, faith community, counseling, follow up with physician, etc.)

    • Documentation of encounter, assessment, interventions, and recommendations

    • Immediate referral for any crisis care per UHC policies/procedures
    • Determination of ongoing care plan needs and scheduling

    • Maintain confidentiality of spiritual care provided to members, their families, and staff, while still advocating, education, and communicating the unique impact of chaplaincy care.

    • Provide both pro-active and responsive holistic support for staff on assigned team.
    • Facilitate staff education and advocacy opportunities on topics relating to spiritual care, such as: inclusion & diversity, employee wellness, and difficult conversations
    • Provide consultation with front-line staff on strategies for specific member/family dynamics, concerns, issues, as they relate to spiritual and emotional/relational support.

    Qualifications:


    • Board-certified as a chaplain through a nationally recognized chaplaincy certification body

    • Five years of experience in professional chaplaincy

    • Master’s degrees in chaplaincy-related studies

    • A minimum of 1600 hours of Clinical Pastoral Education (accredited by the United States Department of Education)

    • More than 200 hours of additional continuing education relevant to chaplaincy

    • Experience dealing with patients diagnosed with complex conditions including, mental and behavioral health conditions, end of life and life limiting disease processes, etc.

    • Flexibility and objectivity to deal with all or no-faith traditions.

    • Agility to be able to navigate consistent changes in structure, processes, priorities, and team.

    • Advocate for evidence-based best practice.

    • Responsiveness to feedback.

    • Ability to articulate alignment of concrete daily provision of spiritual care to the strategic plan, priorities, and need of Complex Health Solutions.

    • Focus care and interventions on the care recipient’s frame of reference and model of meaning-making. With the care recipient, they assess and provide interventions to enhance perceived quality of life (QoL) and overall sense of peace.

    • Computer literacy and a skill level in Microsoft Office Suite of intermediate Word, Excel, Outlook, and PowerPoint.

    • Ability to provide expert and nuanced remote spiritual care via digital platforms such as WebEx and Microsoft Teams.

    • Competency in Outcome Oriented Chaplaincy, capable of clearly communicating spiritual care outcomes, plan of care, and unique impact on spiritual support provided.

    • Substantial remote time spent with members, their families, and staff requiring a high degree of emotional energy and mental alertness.

    • Access to quiet location to make tele chaplaincy calls to UHC Members.

    • Ability to research and connect members with appropriate community resources as needed.
      Chaplain will be issued UHG laptop where all member information must be documented (no personal systems allowed)

    • Chaplains will record note and metrics for each call.

       

    • Master’s degrees in chaplaincy-related studies.
