This care coordinator position will support participants in a new program that targets the remission of type 2 diabetes. Participants will receive insights from technology devices and behavioral coaching to make small changes to behavior including diet, activity, sleep, stress and medications based on data, their social and clinical background and readiness to make changes. Interactions with participants will be via phone, secure messaging and email. Care coordinators will reside in our Virtual Care Center (VCC) located in Minnesota alongside the business leaders and development team. In this role the individual will provide care coordination and intervention activities across the continuum of care in partnership with participants, within the scope of the care coordinators credentials. This includes working in a call center environment and documenting in various systems. This startup role will also contribute to process, documentation and care improvement recommendations. Qualifications include: Active Registered Dietician in the state of operation; minimum of 2 years clinical practice experience in state of operation. A Bachelors (or higher) degree in a health-related field preferred. Certification as a Diabetes Educator preferred. Previous care coordination experience preferred. Individuals will work alongside other care center teammates.
