    Telephonic Case Manager - Contract to Hire Position background

    This position is a telephonic Case Management position, which supports members and their caregivers who are seriously ill , most who are an advanced stage of disease with complex issues. You will be making calls to the members and caregivers assigned to them in their case load to help these member put in place a plan to managing their advanced serious illness. This can include working with caregivers, providers and other team members to have resources and a plan to avoid unnecessary hospitalizations and ER visits.

    Documentation of these calls, discussion and coordination of resources would be documented the same day in an online application uses by our staff for this purpose.

    You would be meeting with your direct manager via telephone or on site depending on the situation to discuss you case load, review program requirements and member specific needs if needed. Weekly attendance of IDT is required and other team meetings as scheduled. The staffing model on this team are teams of RNs and SW who support the membership we serve. We are a supportive team and interact often in the management of the member’s needs. Qualifications:

    Active R.N. nurse licensure in the state of operation (NC); minimum of 3 years clinical practice experience in state of operation.

    A Bachelors (or higher) degree in a health-related field preferred.

    Case management certification preferred. Previous case management experience required.

    Timely management of members needs and case work assigned. Reduction and avoidance of hospital admissions and ER utilization. Participation in Interdisciplinary meetings and collaboration. Shift -  8:30 -5 pm M-F
