A managed care organization has a fulltime CONTRACT position for an experienced Care  manager  /social worker with geriatric experience . This job is remote but you must live near Waltham Mass.

Pay: $23.50

Contract : 6/14 to 10/31

HOURS: 40 a week : 800 AM – 430 PM

LOCATION : Remote and must reside in Massachusetts or surrounding state of Connecticut, Rhode Island or the border of New Hampshire adjacent to Massachusetts.

Qualifications:

• Bachelor degree in Social Services preferred
• Associate Degree (Preferably healthcare or business) required
• Geriatric Social Services or Case Management Experience
• Minimum of 2+ years experience working in case management or a telephonic healthcare environment is required
• Proficient in MS/Office (Word, Excel & Outlook) is required
• Strong typing, writing and grammar skills
• Experience with medical terminology, benefits management, claim and/or healthcare insurance is required

Position Description:
• Providing exceptional customer service
• Multitasking, inbound/outbound phone calls to members and/or physicians
• Completing assessments and documentation within a CER system.
• Provide benefit explanation
• Health care knowledge surrounding disease management, community resources, durable medical/consumable products.

Typical Day:

You will be providing care coordination telephonically completing assessments and documentation within a clinical system while multitasking other applications gathering information to support member’s needs. Must have excellent typing, writing and grammar skills. Primary responsibilities are telephonic case management and some administrative responsibilities for the clinical department related to member care. Very fast paced environment. 10+ people, team player, goal focused, must be a self-motivated, take initiative and willing to learn in a fast environment.
