A large managed care organization has CONTRACT /TEMP positions) that is full-time for an experienced Family Practice Nurse Practitioner who likes doing Health Risk Assessments ( history and physicals) in patient homes in Memphis,Tenn.
Prior home health, telehealth experience was very helpful.
Will accept a new FNP grad with home health experience.
Weekend availability or M-F.

Start Date: after credentialling to December 2023
Hours: Fulltime any day of the week and weekends open too
PAY :
In-Home HRA $100.00 per completed assessment
Travel reimbursed at .655 per mile
No Show paid for in-home $25.00

REQUIRED QUALIFICATIONS :
- Family Practice/Internal Medicine /Primary Care Nurse required with prior Health assessment/hospice or home health experience in patients' homes helpful
- Will accept a new FNP grad with home health experience
- CAQH, and must be credentialled
- Medicare Provider ID and State of the issue required at point of application
- Board certified by the ANCC or AANP
- Must have active/unrestricted license in Pennsylvania
- Required is proficiency with EMR, MS Word, MS Excel, MS Outlook,
- Must have use of own car with adequate automobile insurance
- Highly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basis
- Proven ability to perform comprehensive, adult/geriatric/pediatric/postpartum physical exams
- Demonstrated ability to identify, diagnose and document chronic disease in adults
DUTIES :
- Perform health risk assessments which are essentially history and physicals done within the patient’s home
- Clinical documentation of acute, chronic, and long-term healthcare issues through patient encounters in the EMR system; Documentation must be accurate, high quality, and completed in a timely manner
- The provider will ensure all appropriate consent forms are signed and dated
- Provider will have frequent interaction with patients, coders, schedulers, and supervising physician
- The provider will educate patients and/or patient’s families on chronic medical conditions, preventative care, and medication adherence compliance
- The provider will complete referrals to case management, nursing, pharmacy, etc. as needed
- The provider will not be required to prescribe medicine nor order any lab or diagnostic testing
- Provider will be responsible to review patient records for past and present health issues
- The provider will need to collaborate with the supervising physician and other members of health care team
- part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation
