Job Title: Community Connect County RN- REMOTE
Duties & Responsibilities:
Performs marketing and networking tasks within the healthcare community
Interview, Hire, Train CHWs
Partners to supervise the team of CHWs
Assists the CHWs to coordinate health care between member and the larger care team and facilitate health system navigation
Support CHWs to assess and identify barriers to care; determine goals, objectives, and potential alternatives to care.
Collaborates with Care Team in the development of an integrated care plan that identifies key patient problems, goals of care, barriers to meeting goals, interventions, activation level, and patient self-management activities
Assists with Transitions of Care activities
Assists with maintenance of a patients’ medication record
Provide education to assist patients with self-management of chronic disease, and healthy lifestyle options.
Communicate closely with other healthcare providers so that primary and specialty care is well coordinated, including the use of virtual consult platforms
Ensure Provider is aware of any urgent needs.

Requirements:
Graduate of an accredited medical school
Public health, business administration, or health administration studies preferred
Board Certified in Internal Medicine, Medicine/Pediatrics, or Family Practice
Additional board certification in Emergency Medicine, Gerontology, or Palliative Care is preferred
Experience delivering care to complex patients preferred, including in the home and skilled nursing facility settings
Recent or current experience as a hospitalist physician preferred
Strong knowledge of best clinical practices, particularly in the urgent and post-acute care settings
Must be skilled in delivering care across medical, behavioral, and social needs, particularly for patients living with mental illness and substance use disorders
Demonstrated ability to collaborate both internally and externally with multidisciplinary care team members
Active medical license in the indicated State, or ability to obtain one within 90 days Medication-assisted treatment certification preferred (if not, expectation will be to become certified within first 120 days)
Must have current state and federal DEA certification
Current Certification in AHA or ARC Basic Life Support for healthcare providers required for employment
Working knowledge of computers and ability to document effectively and efficiently in an electronic medical record system
Drivers license requirements: licensed for a minimum of 3 years without restrictions; no DUI or other felony driving conviction in the past 7 years
