    Position Summary: The Case Manager is responsible for the oversight of the health and safety of the individuals supported in the Shared Living and Outreach residences. The Case Manager provides leadership and guidance to ensure the residences operate within all state regulations as well as agency policy and procedures.

    Responsibilities:

    ● To Ensure the health and safety of the individuals at all times

    ● Ensure that the programs operate in accordance with all state regulations and agency policy and procedure

    ● Coordinate and facilitate monthly meetings with Individuals in their homes

    ● Assist individuals with accessing benefit programs as applicable

    ● To ensure that each home is clean, organized, and reflective of the needs of the individuals

    ● To supervise the implementation of individuals’ support plans

    ● To assist in the development of assessments prior to individual team meetings

    ● To ensure that teaching aspects of the program are accomplished

    ● To schedule, coordinate, and provide/arrange for transportation of individuals to appointments/ activities

    ● Ensure that individuals have access to healthy meals in accordance with nutritional standards

    ● To ensure that all individuals have the opportunity to participate in community and social events/activities

    ● To ensure that the programs maintain current home inspections & assist with communicating maintenance needs timely

    ● To ensure that all direct care staff receive adequate training prior to working with individuals independently

    ● To ensure that all staff providing supports maintain current mandatory certifications

    ● To maintain documented and secured supervision notes

    ● To provide ongoing and routine feedback of staff performance and development

    ● To participate in routine supervision and meetings as instructed with the Director of Community Supports

    ● To ensure that funds, medication, and agency property are safeguarded in accordance with agency policy

    ● To maintain clear and accurate documentation of progress notes, communication books, and other program and individual data.

    ● Maintain program schedule in accordance with contracted hours and individual’s needs

    ● Maintain current mandatory certifications to include, not limited to First Aid/CPR

    ● Responsible for safely transporting individuals in agency vehicles as well as personal vehicles in the absence of agency vehicles

    ● Communicate Needs of individuals with appropriate parties

    ● To perform other duties as assigned by the Program Director

    | MINIMUM QUALIFICATIONS H.S. Diploma At least one year of experience in human services Knowledgeable of DDS/DMH regulations Must have valid driver’s license | MENTAL DEMANDS Excellent communication skills Skilled in crisis management and de-escalation | PHYSICAL DEMANDS Bending, stooping, sitting Standing, Lifting
