What is ACT:
ACT is an Evidence Based Practice that is person-centered, recovery-oriented, and a highly intensive community-based service for individuals who have serious and persistent mental illness. The service utilizes a multidisciplinary mental health team, including Psychiatrists, Nurses, Social Workers/Counselors, Substance Abuse Counselors, Employment Specialists, Certified Peer Specialists, and Case Managers. The ACT Team works as one organizational unit providing community-based interventions that are rehabilitative, intensive, integrated, and stage specific. Services emphasize social inclusiveness though relationship building and the active involvement in assisting individuals to achieve a stable and structured lifestyle. ACT is a unique treatment model in which the majority of mental health services are directly provided by the ACT program in the recipient’s natural environment. ACT services are individually tailored with each individual to address his/her preferences and identified goals, which are the basis of the Individualized Recovery Plan.
About Your Role: Our Therapist works as a member of the ACT team, advocating and working with your assigned adult clients. The team is comprised of several members, each with their own focus including housing, vocational services, therapy, medical, and substance use. In your role as Therapist, you will conduct the initial behavioral health assessment and engage the client in development of a treatment plan noting their strengths, challenges, and support systems. You will meet with the client regularly, providing individual, family, and group counseling. and regularly assessing their strengths and mental health treatment needs. This position includes client transportation as needed and one week on-call rotation every 6 weeks, handled virtually or in person, depending on the situation. For optimal client success, you will meet with clients in the local community primarily.
