    About your Role:

    · Under immediate supervision, the Case Manager provides support as it relates to the individualized service plan.

    · Trains and assists consumers with treatment goals.

    · Makes home visits, transports to and from appointments, teaches community living skills such as grocery shopping and paying bills.

    · Teaches self monitoring of psychiatric systems to facilitate coping with illness.

    · Links and teaches consumers to utilize resources within the community.

    · Coordinates needed services for consumers in the community.

    · Monitors consumers in community/ residential/ home settings.

    · Observes, documents and reports consumer behavior/progress on an ongoing basis.

    · Coordinates medical and mental health appointments and works with local resources, consumer families and government agencies such as but not limited to: social security, DFACS, probation and parole.

    · Keeps all progress notes and billing up to date and turned in a timely manner.

    · Adheres to all regulations set forth by policy and procedures.

    Does the Following Apply to You?

    · Must be able to communicate effectively both orally and in writing.

    · Have an understanding of sensitivity to serving the culturally diverse and specials needs population.

    · Have an understanding of and sensitivity to serving the culturally diverse and special needs population.

    · Possess knowledge of the populations served in the services provided.

    · Must possess a minimum of a high school diploma or equivalent.

    · Some college in field of psychology, social work or related field preferred.

    · Some experience in field.

    · Must have clean Motor Vehicle Record (MVR), Criminal Background Check, Drug Screen and physical.

    · CPR/First Aid certification required.

    · Valid Driver’s License.
