    JOB SUMMARY:

    Reporting to the Market General Manager, the Licensed Clinical Social Worker (LCSW) is a key member of the ArchWell Health care team. LCSWs will provide behavioral health services to ArchWell Health members facing complex health and social challenges, driving all psychosocial aspects of the member’s care plan in close collaboration with the entire care team. S/he will assess the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based treatments in center.

    YOU WILL:

    • Collaborate with the primary care teams to conduct behavioral health assessments to determine the appropriate level of care for each member; screen and access each member for common mental health and substance use disorders
    • Develop care plans and provide short-term counseling or facilitate referrals, including warm hand offs for telehealth visits or referrals to external specialists
    • Develop relationships with primary care teams and consistently communicate changes in behavioral health care plans
    • Accurate and timely documentation of patient encounters and counseling sessions in all clinical management systems
    • Proactively identify methods to improve ArchWell Health’s approach based on feedback and regularly conducted surveys

    REQUIRED SKILLS/ABILITIES:

    • Experience supporting patients with co-occurring behavioral health and substance abuse issues
    • Experience with screening, assessment, and treatment planning for common mental health and/or substance use disorders
    • Working knowledge of differential diagnosis of common mental health and/or substance use disorders
    • Highly skilled in behavioral therapy, cognitive behavioral therapy, dialectical behavioral therapy, relapse prevention therapy, contingency management, and motivational interviewing
    • Strong interpersonal communication skills with exceptional active listening abilities
    • Highly empathetic, non-judgmental, and open-minded
    • Experience in a collaborative team environment

    EDUCATION/REQUIREMENTS:

    • Fully Licensed LCSW
    • Master's degree in psychology, counseling, social work, family therapy, or related field
    • A problem-solving orientation and a flexible and positive attitude

    • Proficient PC skills
    • Fluency in Spanish or other languages spoken by people in the communities we serve (where necessary)
