    Outstanding opportunity exists for a Clinical Care Manager to become part of our Community Partner Program! The Community Partner Program is an innovative program that integrates behavioral health with physical health and focuses on “whole person wellness”.

    Under the direction of the Director, the Clinical Care Manager will be part of a dedicated team of professionals and play an essential role in ensuring that clients receive optimal care for both medical and behavioral health conditions.

     Responsibilities:

    • Provides direct and regular supervision of Care Coordinators, including organizational skills and caseload management strategies
    • Responsible for management of referrals, assignments, and dis-enrollments for the CP team
    • Conducts outreach for new referrals which includes verifying insurance and contact information for new members, notifying PCP offices, engaging new members in CP, assigning to Care Coordinators, and monitoring event notifications for emergency room visits and/or inpatient hospitalizations
    • Oversees updates to Person Centered Treatment Plans to ensure that clients are receiving needed services and obtaining goals and that clients have a supportive and responsive care team to help them attain their goals.  
    • Works closely with RN to support the CP team to achieve program goals and quality measures
    • Facilitates weekly team meeting and participates actively in monthly hub management and Clinical Care Manager meetings
    • Responsible for monitoring and maintaining monthly reports, census data, and daily enrollment files
    • Maintain communication with ACO partners and respond to requests in a timely manner
    • Creates an environment where potential safety and clinical risks are identified and proactively addressed before a problem occurs
    • Assures that appropriate information is communicated to all staff and maintains appropriate documentation
    • Provides an environment of quality service that addresses the needs of the client, vendor, community and Aspire Health Alliance
    • Follows the Aspire Health Alliance’s/department policies regarding professional ethics and confidentiality along with actively participating in the Aspire Health Alliance’s Quality Initiatives

    The schedule is Monday through Friday 9am-5pm and includes a $1,000 Sign-on Bonus!

    Must have a clinical license: such as LCSW, LICSW, LMHC, LMFT, LADCI, or LADCII

     

     

     

     

     

     

     

     

     

    Required Skills

    • Excellent communication skills and ability to work successfully and collaboratively with internal and external contacts
    • Demonstrates appropriate autonomy, initiative, and professionalism and the ability and willingness to problem solve as needed
    • Time management and ability to organize priorities
    • Ability to coach, train, and mentor staff and foster an environment of teamwork and professionalism
    • Knowledge of and ability to use Excel

     

    Required Experience

    • Master’s degree in Social Work, Psychology, or a related field
    • Previous experience in providing care coordination for high risk individuals
    • Experience with managing and providing outreach clinical services
    • Valid driver’s license, safe driving record, and access to personal vehicle for outreach and to provide client transportation as needed
    • Must have a clinical license: such as LCSW, LICSW, LMHC, LMFT, LADCI, or LADCII
