Job Title: Associate Patient Care Coordinator (PCC)

Job Summary:
We are seeking a detail-oriented and patient-focused Associate Patient Care Coordinator (PCC) to join our Practice Support Unit (PSU). This position plays a vital role in scheduling annual health screenings, managing patient outreach, verifying insurance, and supporting provider follow-up care. The PCC works primarily from our corporate location and may occasionally assist with patient check-in at health fairs.
Key Responsibilities:
Conduct outbound calls to schedule patients for annual screenings
Verify insurance, demographic details, and patient preferences
Make reminder calls and follow up on cancellations or no-shows
Document all outreach efforts in our system
Support specific PSU providers with appointment scheduling
Respond to and direct patient messages, ensuring timely follow-up
Meet daily productivity and call quality goals
Qualifications:
High school diploma or GED required
Excellent communication and customer service skills
Basic Microsoft Office knowledge
Prior healthcare, EMR, ICD-10/CPT coding, or data entry experience a plus
Physical Requirements:
Ability to sit for long periods and occasionally lift up to 50 lbs
Strong attention to detail and multitasking skills
