About the Role:

Star Light Home Health in Montrose, CO is looking for a passionate and driven Assistant Director of Nursing to join our dedicated care team. Responsibilities:

Assist the Director of Nursing in overseeing daily clinical operations and ensuring high-quality patient care

Supervise, mentor, and support nursing staff and field clinicians to promote best practices

Conduct patient assessments, care plan reviews, and ensure compliance with home health regulations

Monitor and evaluate clinical outcomes, identifying areas for quality improvement

Ensure adherence to Medicare, Medicaid, and state/federal home health guidelines

Participate in hiring, onboarding, and ongoing education of nursing personnel

Collaborate with physicians, therapists, and interdisciplinary team members on patient care coordination

Requirements:

Active RN license in the state of Colorado (or compact license eligibility)

Minimum 2-3 years of clinical nursing experience, preferably in home health or community care

Prior supervisory or nursing leadership experience strongly preferred

Strong knowledge of Medicare/Medicaid home health conditions of participation

Excellent communication, organizational, and critical thinking skills

Experience with OASIS documentation and home health EMR systems a plus
