This position requires a CNA, LPN, or a minimum of 5 years’ experience working in a skilled nursing facility or home care settings with at least one year in a supervisory capacity. Bilingual English/Spanish required.
Conduct home visits with clients to review services, customer satisfaction and ensure delivery of care is appropriate.
Conduct employee evaluations by observing direct care staff during service deliver, review performance with clients and family.
Run reports in home care management software and maintain own schedule for home visits
Notify supervisor of any clients or direct care staff who refuse an evaluation or any issues or concerns that are observed during a home visit.
Complete all documentation required and enter information into software system
We are looking for someone who is interested in learning to be a Direct Care Worker and CPR and First Aid Instructor in addition to the above job duties.
This person must be 18 years of age or older, have reliable transportation, a valid AZ driver’s license and proof of insurance Must have a Level 1 Fingerprint clearance card or be able to obtain one. Must be computer literate with Word, Excel, Outlook, internet and have the ability to learn new software programs. Must have excellent customer service skills, be available to work flexible hours, type, file, sit, stand, bend, and lift at least 25 pounds.
