    Autism Case Manager for Adult Programs

    Fulltime

    Case Locations: Communities in Hampden, Hampshire and Franklin County Massachusetts.

    Pay: $20.00 / Hour + Outreach travel reimbursement.

    Schedule: Monday - Friday, 8 Hours daily, between 9AM-7PM with flexibility to best meet case load needs, adjusting hours as needed. Adult Outreach Services made available throughout Hampden Hampshire & Franklin Counties.

    ServiceNet, a leading Human Services agency, is seeking compassionate, dedicated, and enthusiastic individuals to join our team. As a Case Manager / Autism Specialist, you will work directly with a caseload of adults in their own residences and in local communities with them. Support is focused on goals identified by the individual and their team including household management, executive functioning, social/recreational, health care management, and other areas of life. Adults programs work with a caseload of individuals who, with the right supports, can remain living independently.

    Key Responsibilities:

    Be part of teams that provide outreach support to adults with autism.

    Partner with them in creating and reaching their goals, increasing their skills, and making connections within their local communities.

    COMMON INDIVIDUAL GOALS CAN INCLUDE:

    Living more independently.

    Health Care Coordination.

    Supporting routines such as grocery shopping, meal preparation.

    Care of living space.

    Financial decision making.

    Crisis support.

    Accessing and connecting with the local community, recreation and making friends.

    Day to day problem solving.

    Qualifications:

    Bachelor’s degree preferred or education equivalent to a 4-year college degree.

    Human Services residential outreach experience.

    Knowledge of Autism services and treatments.

    Over 1 year supporting adults with autism.

    Experience in mental health support preferred.

    Reliable vehicle, valid driver's license and willing to transport individuals when needed.
