NYC Early Intervention Service Coordinator
JOB RESPONSIBILITIES Oversight and management of an EI Caseload including, but not limited to: partly remote position, local travel may require sometimes * -Introduce families to the Early Intervention Program -Assist families with comprehending the process and services available for their child/ children. -Secure multidisciplinary evaluations to determine the child's eligibility. -Participate in the development, implementation, and monitoring of the Individualized Family Service Plan -Obtain proper services for eligible children. -Maintain ongoing and consistent contact with families to assure provision of services, correct data for amending services, as well as secure a transition plan out of the Early Intervention Program. -Maintain updated case records, service plan reviews, and document all contacts with families, and service providers in compliance with state regulations. -Obtain completed medical forms and necessary prescriptions from the child's pediatrician. -Sustain ongoing communication with families and service providers. -Attend ongoing training. -Attend meetings with families- home visits, IFSPs, reviews and IEP meetings as scheduled. -Navigated, updated, uploaded Early Intervention paperwork, and entered information into NYEIS, Providersoft and Salesforce. JOB REQUIREMENTS * BS/BA is Psych or A bachelor's degree in a health or human service field. * Bilingual skill is a plus * Experienced in EI Service Coordination a must (at least 1 year) * Strong commitment to helping families * Highly organized with ability to multi task * Ability to work independently and well with others * Must have excellent oral and written communication skills * Willing to work in the field/commute for meetings Competitive and attractive per unite rate. The AEES program adheres to core principles of the national Early Intervention Program for infants and toddlers with disabilities and their families, based on Part C of the Individuals with Disabilities Education Act. Intervention is provided to the maximum extent possible in natural and inclusive environments and integrated throughout typical everyday experiences, where parents play a primary role in their child's learning.
