    TITLE: BA Level Supportive Counselor

    RESPONSIBLE TO: Clinical Director

    WORK AREA: Outpatient sites and Community

    SUMMARY:

    Path's Supportive Counselors are responsible for helping those with chronic illness, mental

    health issues or life-altering conditions to gain access to rehabilitation programs and other

    community services. Their duties include meeting with consumers, connecting consumers with

    professional services and maintaining consistent communication with consumers to assess their

    wellbeing and make changes to their treatment plan as needed.

    Responsibilities and Duties:

    • Assessing Consumers’ physical and mental wellness, needs, preferences and abilities and

    using these to develop tailored care plans

    • Working with Consumers, support networks and health care professionals to put care plans in

    place.

    • Listening to Consumers’ concerns and providing counseling or intervention as required
    • Recording Consumers’ progress including charting referrals, home visits and other notable

    interactions

    • Evaluating Consumers’ progress periodically and adjusting as needed to improve outcomes
    • Following up with discharged Consumers to ensure they were satisfied with services, and they

    are still in good physical and mental health

    • Monitoring consumers over time to access treatment progress.
    • Collaborating with additional resources to provide a well-rounded scope of practice.
    • Identifying behaviors that interfere with treatment and recovery.
    • Assisting primary caregivers to manage addiction-related behavior.
    • Perform Additional duties as assigned.

    Qualifications:

    • A bachelor's degree in Behavioral Science and/or Human Service or related field.
    • Compassionate and able to relate to different Consumers with various needs
    • Motivational to encourage Consumers to follow their care plans
    • Strong verbal and written communication skills to explain to Consumers, family members and

    friends and professionals the case and care plan and maintain good case records

    • Critical thinking and problem solving to determine the best care plan for each Consumer after

    assessing Consumers, analyzing notes from health care and social workers

    • Flexibility to change care plans if they are not getting the best results
    • Organization to manage several different cases at once
    • Computer literacy to maintain and manage case records

    Schedule:

    Monday to Friday

    Weekends as needed
