Requirements: ✔️ Bilingual English/Spanish REQUIRED ✔️ Experience coordinating patients and HHAs ✔️ HHAeXchange experience preferred

Responsibilities:

Coordinate patient care and HHA services

Assist with HHA staffing and schedules

Communicate with patients, families, and caregivers

Follow up on patient needs and schedule changes

Maintain accurate patient documentation

Support daily home health operations
