  • Responsibilities

    DUTIES/RESPONSIBILITIES:

    Develop and implement orientation procedure for incoming clients. Meet with prospective candidates for housing program with program staff. Teach clients and staff entitlements procedures. Make referrals to government agencies and escort clients as needed to procure entitlements. Reconcile all client beneficiary accounts and oversee representative payee accounts. Prepare and monitor all client accounts and keep appropriate records. Provide assistance with securing and maintaining active benefits, including Food Stamps, Social Security, Medicare, Medicaid, Public Assistance, WIC, managed care, and other health insurance. Complete all documentation, record keeping, and progress notes in a timely manner. Responsible for completing forms required by the agency. Develop and facilitate group curriculum around obtaining and maintaining entitlements. Attend staff/team meetings and trainings. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

     

    HOURS:

    Full time, 37.5 hours per week

    • Monday - Friday; 9:00am - 5:30pm

     

    QUALIFICATIONS:

    High School Diploma/GED and a minimum of one year experience working in with entitlements or case management. Bachelor of Arts degree is preferred. Ability and willingness to drive the Home Plus van is preferred. Bi-lingual (Spanish) preferred.

    PROGRAM DESCRIPTION:

    BRC maintains 390 supported apartments throughout Manhattan, Brooklyn and the Bronx through the HomePlus program. These apartments are dedicated for formerly homeless individuals with a history of mental illness and/or substance abuse, and are funded by a variety of state and city mental health agencies, including the NYS Office of Mental Health, the NYC Department of Health and Mental Hygiene, the US Department of Housing and Urban Development, and the NYS Office of Alcohol and Substance Abuse Services. BRC's HomePlus operations have grown significantly in recent years, including 131 units for substance abusing, chronically homeless individuals that were brought on line in 2008 and 65 units of scattered-site housing for formerly homeless individuals with a history of mental illness that were brought on line in 2009 and 2010.
