    Broader Urban Involvement and Leadership Development (BUILD, Inc.) is seeking a committed, self-motivated, enthusiastic professional to join the Clinical and Community Wellness Services team in the role of Mobile Community Wellness Advocate (CWA). Since 1969, BUILD has worked in some of Chicago's toughest neighborhoods giving at-risk youth alternatives to the violence that takes away their positive potential. Applying our model of positive youth development, BUILD seeks to redirect the behavior of gang-affiliated youth and potential gang recruits to improve their chances of leading fulfilling lives and to increase the stability and safety of the communities in which they reside. The Mobile Community Wellness Advocate will provide emotional support and services to youth and their families who are victims of crime and/or homicide through the process of assessing, planning, implementing, coordinating, monitoring and evaluating options and services required to meet the client's service plan needs and goals. Through advocacy, communication and resource management, the Community Wellness Advocate will promote equality and cost-effective interventions and outcomes helping find services needed and keeping tabs on the client's progress through the completion of their service plans. Services include but are not limited to: connecting clients with additional critical resources, such as mental health care, medical care, legal services, victim's rights and/or compensation, funeral planning and vigils, interpretation services, workforce development and financial education. Basic needs such as emergency financial assistance, childcare, transportation, housing and utilities are also included. Emotional support navigating the criminal justice system, including police departments, legal services and the courts. Services take place at BUILD, in the community and other locations as needed . The Mobile Community Wellness Advocate reports directly to the Mobile Mental Health Program Manager Responsibilities include: Direct Service: Advocates for victims of crime and their immediate families in problem situations, including appeals, reconsiderations, and attempts to meet family needs through alternative resources when Connect violence-exposed youth and their families with comprehensive services that promote healing, foster life-skills, and prevent high-risk/consequence behaviors that make them vulnerable to violence and other negative outcomes Documentation and Competencies: Contributes to client related data systems for record keeping and reporting purposes Contributes to reports as required per grant Conducts assessment of client's immediate needs and circumstances to develop an appropriate service plan Ensure the timely flow of case management activities to meet program service plan goals Utilizes BUILD Universal Referral form to document and connect families to other BUILD services as needed Operates professionally in the field on a flexible schedule, which may include late evenings and weekends Completes all assessments, service plans, case notes and any other significant program data within a timely manner Maintains all forms required, including case notes, client service plans, referral forms, information releases related to health or disabling conditions, health information, and any other information necessary in program participants file utilizing all procedures related to maintaining confidentiality at all times Communication and relationship management: In partnership with Crisis Response Specialist and Community Wellness Navigator, helps to identify positive adult/youth influences to create a stronger support network for the program participant Regularly monitors client activity related to achieving personal and programmatic goals, and provides revisions to service plans as needed Establishes a regular schedule of home visits, contacts, and follow-ups to encourage the building of relationships with participants and ensure completion of service plans Utilize motivational interviewing practices to engage and retain participants through the completion of their service plans Represents BUILD to broaden community awareness and support, interpreting the agency's programs and services at program-related community, coalition and committee meetings as needed Attends departmental and agency-wide staff meetings, as scheduled and agency-wide events, as required Maintains knowledge current in the field in way of participation of coursework, seminars and/or workshops Other: Participate in agency-wide fundraising events Participate in agency outreach and recruitment events, including evenings and weekends Participate in civic engagement activities that advance the mission of BUILD, Inc. Perform other duties as assigned Qualifications: Minimum two years of experience working with inner-city youth in a human service setting, trauma-informed care, social service setting, or related field Bilingual- English/Spanish preferred Must demonstrate excellent interpersonal, verbal and written communications skills An automobile is required along with current insurance and a valid driver's license Maintain respectful and appropriate relationships with program participants, all levels of agency staff and the various publics of the agency, demonstrating sensitivity to all ethnic, racial, and economic backgrounds Must be willing to travel as needed Must be passionate about youth development work Must successfully pass extensive background check Must provide proof if COVID-19 Vaccination This organization reserves the right to revise or change job duties and responsibilities as the need arises.
