JOB DESCRIPTION
The Baltimore City Health Department, Local Health Improvement Coalition is seeking a Fellow to support the work of developing two key strategies, as part of a 5-year CDC grant initiative. The Fellow will lead, under the guidance of the LHIC director, 2 key strategies to address the structural barriers to diabetes prevention. The work includes developing a strategy to improve referrals to social needs and addressing the social determinants of health for people at risk for diabetes. Note, this is a 1 year position and will have the opportunity to renew, depending on funding.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Data Collection & Analysis (30%)
Collaborate with our team to design and execute surveys, interviews, and data collection methods to gather information on social determinants of health and referral systems.
Conduct interviews with key stakeholders
Review and summarize relevant literature
Report Development (20%)
Take a leading role in summarizing key findings, in collaboration with data manager and program director
Develop a communication plan to share out results
Implementation Plan and Recommendations (20%)
Develop detailed implementation plans based on assessment outcomes, ensuring alignment with organizational goals and objectives.
Collaborate with cross-functional teams to define project scopes, timelines, and resource requirements.
Stakeholder Engagement (30%)
Convene community stakeholders to gather and share information
Convene stakeholders to develop implementation plans
Participate in LHIC (Local Health Improvement Coalition) meetings and contribute to agendas, etc. as needed
Participate and contribute in external convenings and coalitions as needed
REQUIRED QUALIFICATIONS
Bachelor’s degree with interest in public health, health equity, health education, or health or public policy.
Excellent verbal and written communication skills
Self-starter with strong follow through, solutions-oriented and has great attention to detail
Ability to work in a fast-paced in environment
Passion and interest in community health, health equity and diabetes
PREFERRED QUALIFICATIONS
Knowledge of landscape assessment, implementation planning
Willingness to learn and adapt in a dynamic work environment
