    The Opportunity

    Community Health Pathways (CHP) is a Baltimore Corps Public Health AmeriCorps program. Baltimore Corps enlists talent to accelerate social innovation in Baltimore and advance a citywide agenda for equity and racial justice. By matching talent to opportunity, Baltimore Corps works to empower the social entrepreneurs, community leaders, and public sector innovators tackling some of the city’s most pressing challenges and building a stronger Baltimore every day.     CHP members will serve as Community Health Navigators, increasing access to and quality of care for Baltimore’s underserved communities. Community Health Navigators will serve at a Baltimore-based community health organization liaising between health care providers, social services, and patients. Members will be responsible for building trusted relationships with community members and providing services through various activities, including but not limited to outreach, follow-up, peer support, and resource referral. After a year of service Community Health Navigators join the Baltimore Corps network of changemakers and will be prepared to launch into full-time healthcare careers at top hospital employers with the necessary health certifications provided by the program.

    Community Health Navigator, CNA - Increase access to and quality of care for Baltimore’s underserved communities. Community Health Navigators serving at a Baltimore-based community health organization will liaise between health care providers, social services, and patients.

    • Certification: A Certified Nursing Assistant (CNA), provides health care for clients and patients at home, hospitals, and other facilities under the direction of a licensed nurse. They often help people under their care perform day-to-day tasks and play a vital role in hospitals and other facilities.

    Community Health Navigator, CHW - Increase access to and quality of care for Baltimore’s underserved communities. Community Health Navigators serving at a Baltimore-based community health organization will liaise between health care providers, social services, and patients.

    • Certification: A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of the community served. This trusting relationship enables a CHW to serve as a liaison to, link to, or intermediary between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

    Community Health Navigator, CPRS - Increase access to and quality of care for Baltimores ’historically and intentionally excluded communities by providing recovery support services to community members living with and history of behavioral health disorders.

    • Certification: A Certified Peer Recovery Specialist (CPRS) uses their lived experience to support and promote recovery and prevent relapse offering social emotional and/or educational supportive services to help prevent relapse and promote recovery.

    Community Violence Navigator, CHW - support healthcare staff and community members in implementing trauma-informed care strategies and providing safety planning and follow-up services to violence-impacted individuals. Navigators will guide affected community members to the wraparound services offered by local community partners & affiliates.

    • Certification: A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of the community served. This trusting relationship enables a CHW to serve as a liaison to, link to, or intermediary between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

    Community Resource Connector (CHW or CPRS) – Help community members access the resources and services they need to thrive

    • Certification: A Certified Peer Recovery Specialist (CPRS) uses their lived experience to support and promote recovery and prevent relapse offering social emotional and/or educational supportive services to help prevent relapse and promote recovery.

    What is AmeriCorps? What is a service year?

    AmeriCorps is a national service organization dedicated to strengthening communities through service and volunteering. Community Health Pathways (CHP) is a Public Health AmeriCorps program operated by Baltimore Corps.

    A service opportunity is a great way to launch your career! This AmeriCorps service opportunity is up to an 11-month commitment that offers supported learning opportunities and real-time experience in the workplace. Participating in a term of AmeriCorps service gives you a unique opportunity to develop professionally and personally and contribute to on-the-ground efforts that work for and with communities.

    CHP AmeriCorps members will receive a living stipend of up to $30,000 over 11 months or up to $15,000 over 6 months, depending on the chosen pathway to cover their costs of living while completing their service term. In addition to the living stipend, AmeriCorps members receive health benefits, supplemental nutrition (SNAP)*, child care credit*, transit benefits, and up to a $7,395 Segal (11 months) or up to $3,697.50 (6 months) Education Award upon completing the service year, which may be used to pay for college loans or tuition.

    CHP AmeriCorps Members will comply with AmeriCorps regulations and will not engage in AmeriCorps Prohibited Activities while serving or representing Community Health Pathways.

    AmeriCorps Member Position Requirements:

    • Be a high school graduate or GED recipient or equivalent

    • 18 years old or older

    • Hold one of the following citizenship statuses: US citizen, US National, Lawful Permanent Resident Aliens

    • Pass a National Service Criminal History Check (AmeriCorps requirement)

    • Covid-19 vaccinated

    You’ll be a great fit if you have the following:

    • A desire to work with diverse and underserved communities and a true desire to help people improve the quality of their lives

    • You enjoy learning and using a variety of communication skills (one-to-one speaking, public speaking, and digital sharing such as emails)

    • A desire to serve in Public Health, Social Work, Human Services, Case Management, or related areas

    • Ability to relate well to people from different backgrounds

    • Experience in MS office, Google Suite, and computer use
