JOB DESCRIPTION
Provides case management services to older adults in Baltimore City.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Case Management (100%)
Provide program information including eligibility requirements to individuals and/or family members who are interested in the state programs
Conduct scheduled phone contacts or home visits with clients
Provide assistance with referrals, verify financial information, determine eligibility and the services
Maintain a client waiting list when necessary
Maintain case files for each client and track client expenditures
Annually re-evaluate client financial and medical eligibility
Maintaining case records and notes
Maintaining appropriate data in various data tracking systems
REQUIRED QUALIFICATIONS
Bachelor’s Degree in Social/Human Services or a related field from an accredited college or university.
Minimum of two (2) years’ experience in human services or related field.
