  • Responsibilities

    The Options Counselor Caregiver Support is responsible for managing all aspects of a client’s healthcare maintenance and treatment. Their duties include helping clients complete paperwork, communicating with a healthcare team about a client’s treatment plan and educating patients about resources and options for managing their health. This position reports to the Lead Caregiver Support Options Counselor. Salary $45,000/yr to $55,000/yr

    ESSENTIAL FUNCTIONS
    • Determine needs and evaluate interventions. Assist with diagnosis and planning.

    · Guide individuals of varying backgrounds towards a common goal for the betterment of a client’s health and quality of life.

    · Develop and coordinate dementia care programs.

    · Develop support groups and dementia friendly communities to friends, families, and businesses.

    · Creates and develops advisory committee for the Middle Peninsula amp; Northern Neck.

    · Provides education, resources, and caregiver support.

    · Communicate between staff, clients, and family.

    · Handle client case management and education.

    · Develop client goals and monitoring progress.

    · Maintain client confidentiality and quality care.

    KNOWLEDGE, SKILLS, AND ABILITIES
    • Superb communication skills.

    · Ability to work effectively with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner.

    · Experience developing and delivering client specific counseling.

    · Knowledge of community and community resources and ability to efficiently refer to same.

    · Ability to develop case plans and case summaries and record comprehensive case notes.

    · Strong clerical/administrative skills including personal computer skills (Microsoft Office software).

    · Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.

    · Knowledge of electronic medical records.

    · Exceedingly organized with strong attention to detail.

    · Demonstrated oral/written skills.

    • Proficient in Microsoft Office Suite

    MINIMUM QUALIFICATIONS
    • Associate or bachelor’s degree combined with continuing education credits in geriatrics,social service, leadership or related, strongly preferred. Experience may substitute.

    · 2+ years of experience in caregiver support strongly preferred. 2+ years of experience planning, coordinating and leading activities and events.

    WORKING ENVIRONMENT **
    ** Office-based position with minimal physical requirements (ability to lift 15lbs, walking and climbing stairs).

    FLSA status: This is an exempt position.
