    JOB SUMMARY

    Maintain the highest level of quality of life and independence possible for the Adult Day Break (ADB) clients while providing respite to care givers and families. Ensure the daily activities of Center are within the regulations set forth by State and Federal agencies (e.g. DSS, DMAS, VA).

     ESSENTIAL FUNCTIONS

    1.     _ CENTER ADMINISTRATION AND BAY AGING PROGRAM FUNCTIONS_

    -         Responsible for maintaining clients’ records in accordance with all state and federal licensing agencies (Department of Social services, Department of Medical Assistance Services, Veterans’ Affairs).

    -         Ensure the center follows guidelines for food preparation and is in compliance with Health Department and fire regulations.

    -         Produces monthly reports on Center’s activities for Bay Aging management.

    -         Assist with other Bay Aging program functions that require Registered Nurse oversight and function to include the Home Care and Care Coordination departments. 

    2.     _ADB CENTER OPERATIONS _

    -         Oversee daily operations in the Center (e.g. set-up, clean up, care, activities, meal preparation, and transportation).

    -         Establish individual care plan and annual follow-up for ADB clients. Monitors blood pressure and weight. Administers medication based on prescripions.

    -         Plan weekly menus. Handle the procurement of food and supplies for the Center.

    -         Assist with care activities if short-staffed.

    3.     _CUSTOMER SERVICE _

    -         Provide tour of facility to family members and caregivers inquiring about the ADB Center.

    -         Maintain regular contact with clients’ families. Listen to and assist with resolution of families’ concerns, complaints and questions.

    4.     _ABD STAFF MANAGEMENT _

    -         Hire, train and supervise ADB Aides.

    -         Establish work schedule to ensure appropriate coverage and care in the Center and during transportation to and from the Center daily.

    5.     _MARKETING _

    -         Attend local community meetings related to senior care/safety.

    -         Participate in local events targeting seniors (e.g. Senior Fairs).

    -         Create events such as an Open House to raise the awareness of the Center within its service area.

    -         Establish relationships with potential referral sources such as convalescent centers and chapters of the Alzheimer’s Association.

    _ _DECISION MAKING

    Perform a variety of tasks independently. Involve supervisor when difficult decisions have to be made regarding the placement of clients, and in case of large purchases and capital expenditures.

     KNOWLEDGE

    Has knowledge of:

    -         Federal and state regulations related to adult care centers (DMAS, DSS, VA, Health department)

    -         HIPAA regulations relevant to ADB center operations.

    -         Microsoft Office programs (e.g. word processing, spreadsheet, e-mail).

    -         Bay Aging corporate policies and procedures related to Adult Day Break Centers.

    Stay current in the knowledge of the profession by completing at least 24 hours of in-service per year.

     ABILITIES

    -         Take charge for all aspects of the Center operations and building maintenance.

    -         Communicate in small groups and one on one with a variety of people from medical professionals to caregivers and family members.

    -         Be flexible and creative to adapt to changing circumstances.

    -         Establish and maintain caring relationships with clients and their families even in difficult circumstances.

    -         Establish and maintain relationships across the community to raise the profile of the ADB Center and its services.

    -         Supervise and counsel ADB staff to maintain professional, caring and safe environment in the Center at all times.

    -         Ability to stand, bend, stoop, pull, push and lift up to 25lbs to assist with loading and unloading in transportation.

     WORKING ENVIRONMENT

    The position is based at an Adult Day Break Center. Require regular independent trips in the local community. None of the essential functions of the position can be performed virtually.

     QUALIFICATIONS

    -         Bachelor’s degree in the medical field or human services.

    -         Have 2 years of experience working with seniors or disabled adults.

     Must possess a valid driver’s license and proof of current PPD vaccination.

     FLSA status: This is an exempt position.
