    Bay Aging is seeking a part-time Care Coordinator in Montross, VA. As a part of a multidisciplinary team, the Care Coordinator will provide direct services including individual, therapeutic services to clients, such as assessments, care plans, and discharge planning. This position reports to the Director, Community Living Programs. ESSENTIAL DUTIES Primary duties include: • Addresses referrals from all sources. • Participates and assesses daily needs of clients, coordinates services, and enters into database. • Maintains weekly scheduled appointments in our electronic records systems. • Provides therapeutic services to clients and their families. • Works in collaboration with multidisciplinary team members including external partners. • Completes required documentation and paperwork including assessments and care planning and implementation in a timely manner. • Demonstrates cultural competency and commitment to patients. • Demonstrates commitment to the philosophy of the agency. • Maintain case load appropriate to current workload • Other duties as assigned. Secondary duties: • Assist with functions necessary to maintain NCQA (National Committee Quality Assurance) accreditation, including: o Works with all departments to assist with reviewing, revising, and preparing policies, protocols, and procedures for NCQA credentials. o Assists with training to the team regarding NCQA process. o Works with organization to ensure ongoing compliance with NCQA standards and submits relevant reports. SUPERVISORY RESPONSIBILITY The incumbent in this position may have supervisory responsibility. They will be expected to provide leadership and instruction to colleagues and volunteers. ENVIRONMENT The environment for this position will be in the clients’ homes and in an office setting. In addition, the incumbent maybe required to travel via personal vehicle to client homes. PHYSICAL ACTIVITY In the course of performing this work, the incumbent: • Will spend substantial time standing, sitting, speaking and listening. • Must regularly lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job. QUALIFICATIONS/POSITION REQUIREMENTS These specifications are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this job. Individual abilities may result in some deviation from these guidelines. • Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner. • Experience developing and delivering client specific counseling. • Knowledge of community and community resources and ability to efficiently refer to same. • Ability to develop case plans and case summaries and record comprehensive case notes. • Strong clerical/administrative skills including personal computer skills (Microsoft Office software). • Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures. • Knowledge of electronic medical records. • Exceedingly organized with strong attention to detail. • Demonstrated oral/written skills. • Working knowledge of NCQA regulations and requirements, preferred.
