    POSITION SUMMARY

    A Bayless Community Health Engagement Specialist is a patient supporter working within Bayless Integrated Healthcare and the community settings. Building rapport and trust are two key objectives. The Specialist helps contact patients and connect them to all needed medical, psychiatric and psychotherapy services. This position supports medical providers and patients to fully integrate into Bayless’s treatment specialties. 

    Responsibilities of a community health engagement specialist include assisting patients in medical referrals and follow-ups. Establishing a close relationship with the patient is necessary for good integrative care. Exhibiting excellent communication skills virtually and in person with the members, patients, staff, providers, and visitors should be the guiding principles for effective community health engagement.

    PRIMARY JOB RESPONSIBILITIES:

    • Outreach to defined roster patients through a variety of mediums. 
    • Documents patient progress via EMR and outreach activities
    • Assess patients initial care needs
    • Assigns patients to clinical measures reporting platform
    • Assists in monthly roster reconciliation and outreaches to new assigned patients
    • Documents actions and outcomes of outreach in EMR, reports, and portals as required
    • Track/trend rosters and assignments
    • Collect and review medical records and supporting documentation, determine completeness and accuracy as it relates to clinical outcomes and value based contracting specifications

     

    QUALIFICATIONS The most important qualification is an ability to engage and communicate with members needing care.  An equivalent combination of education, training and experience in non-medical fields will be considered.

    • Community Health Worker Certification or AA degree in health-related field, business, sales or public administration, psychology, organizational behavior or a closely related field.

    • At least 1 year of experience in health care, public health, business, human resources, workforce development, public administration, marketing/sales, or a related area.

    • Knowledge of the Maricopa County health issues, health equity and disparities.

    • Proficiency in Microsoft Word, Excel, and PowerPoint

    • Exceptional communication and time management skills

    • Demonstrated ability to provide leadership and work independently

    • Bilingual English/Spanish communication skills helpful

     

    TOOLS AND EQUIPMENT REQUIREMENTS

    • The ability to use a phone, computer, printer, and copier is required.
    • The ability to coach members how to use technology to connect to telemedicine appointments
    • Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint. 
    • The ability to use the internet and various web browser software is required.

     

    PHYSICAL WORKING CONDITIONS AND OFFICE SETTING DESCRIPTION

    • Travel required to field locations to facilitate execution of responsibilities. Must provide proof of a valid driver’s license, valid auto insurance with, at minimum, state required insurance levels. 
    • Requires prolonged sitting, standing, frequent bending, stooping, or stretching. 
    • Some lifting may be required.
    • Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.
    • Requires occasional use of fax machines, telephones, copiers, and other office equipment.
    • Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and or move up to 50 pounds.
