Jennie Stuart Health is seeking a BHU Social Worker to join our Behavioral Health Unit. This role is responsible for supporting patients through psychosocial assessment, treatment planning, therapy participation, and discharge coordination. The BHU Social Worker works closely with the interdisciplinary Treatment Team to help ensure patients receive appropriate social services and follow-up care throughout their stay and at discharge.
The ideal candidate will have a strong background in behavioral health or healthcare social work, excellent communication skills, and the ability to work collaboratively in a fast-paced clinical setting.
Essential Responsibilities:
- Conduct psychosocial interviews and gather social history information relevant to treatment and discharge planning
- Assess social factors affecting the patient’s condition and modify service plans as needed
- Participate in Treatment Team meetings and contribute to patient care planning
- Meet with patients and families in individual or group settings as requested
- Conduct group therapy and support groups as needed
- Serve as the Discharge Planner and coordinate appropriate social services and follow-up care
- Complete discharge-related forms and documentation accurately and timely
- Ensure compliance with applicable CMS, Medicare, and state licensure requirements
- Collect and compile data, complete reports, and support quality improvement initiatives
- Collaborate with hospital staff and outside resources to support continuity of care
Qualifications:
- Master’s degree in Social Work from an accredited Graduate School of Social Work required
- Social Worker licensure required
- CSW or LCSW preferred
- CPR certification preferred
- Experience in psychotherapy, discharge planning, and social services in a healthcare setting preferred
- Behavioral health or psychiatric care experience strongly preferred
- Knowledge of Medicare, Medicaid, insurance requirements, medical terminology, and community resources preferred
