Are you passionate about enhancing the quality of life for older adults through behavior management, fall prevention, and compassionate care? Join our dynamic team in Tracy, CA as a Behavior Support Coordinator and play a critical role in ensuring resident safety, dignity, and well-being.
The Behavior Support Coordinator leads and oversees innovative programs such as the Turning Point Initiative and Falling Star Program , designs person-centered behavior support plans, and ensures regulatory compliance with Title 22 and DHCS standards. You’ll work collaboratively with care teams, train staff on behavior and fall protocols, and support residents and families through effective communication and engagement.
Develop and manage behavior and fall prevention programs
Conduct behavioral and fall risk assessments
Collaborate with interdisciplinary teams including caregivers, care coordinators, and enrichment staff
Supervise the Behavior Support Specialist and support staff training
Ensure regulatory documentation and compliance (ALIS, CCLD, DHCS)
Engage families in care planning and education
Respond to behavioral emergencies and adjust interventions
Analyze data to improve resident outcomes and program effectiveness
Bachelor’s in Psychology, Social Work, or related field preferred (or equivalent experience)
5+ years in elder care, behavioral health, or fall prevention
Deep understanding of Title 22 and DHCS guidelines
Strong leadership, assessment, and communication skills
Experience with behavior contracts and person-centered interventions
Must pass background and health screenings per California regulations
CPR/First Aid Certified, TB Clearance, LIC 503/508 required
Previous experience in assisted living or as a medication assistant
Familiarity with ALIS and care coordination platforms
Ability to lead team huddles and staff development sessions
Able to lift up to 40 lbs. and remain on feet for extended periods
Must be at least 18 years old with reliable transportation
Exposure to blood/bodily fluids possible (universal precautions apply)
