    Position Overview

    We are seeking a dedicated full time Case Manager to join our team. This role is ideal for someone passionate about helping others, with experience in behavioral health and substance abuse treatment. You’ll play a key role in coordinating care, facilitating groups, and supporting clients through every stage of their recovery.

    Key Responsibilities

    • Conduct client intakes and assessments

    • Develop and manage individualized service plans

    • Assist with FMLA and Short-Term Disability documentation

    • Coordinate medical, psychiatric, and legal appointments

    • Facilitate group sessions and provide direct client support

    • Monitor client progress and update treatment plans

    • Maintain accurate documentation in KIPU EMR within 24 hours

    • Collaborate with healthcare providers, community partners, and community agencies

    • Participate in weekly interdisciplinary team meetings and supervision

    • Ensure compliance with ADHS and Joint Commission requirements and company policies

    • Build and maintain professional relationships with referring agencies

    Qualifications

    • High School Diploma required; Bachelor’s or Master’s degree preferred

    • Minimum 1+ year of case management experience

    • At least 1 year of experience in substance abuse or behavioral health

    • Valid driver’s license and reliable transportation

    • AZDPS Level 1 Fingerprint Clearance Card

    • Current CPR/First Aid certification

    • Preferred: PRSS certification and behavioral health coursework

    Physical Requirements

    • Ability to walk, bend, stand, and climb stairs

    • Lift up to 25 lbs occasionally

    • Communicate effectively in English
