Behavioral Health Case Manager

Position Overview
We are seeking a dedicated full time Case Manager to join our team. This role is ideal for someone passionate about helping others, with experience in behavioral health and substance abuse treatment. You’ll play a key role in coordinating care, facilitating groups, and supporting clients through every stage of their recovery.
Key Responsibilities
Conduct client intakes and assessments
Develop and manage individualized service plans
Assist with FMLA and Short-Term Disability documentation
Coordinate medical, psychiatric, and legal appointments
Facilitate group sessions and provide direct client support
Monitor client progress and update treatment plans
Maintain accurate documentation in KIPU EMR within 24 hours
Collaborate with healthcare providers, community partners, and community agencies
Participate in weekly interdisciplinary team meetings and supervision
Ensure compliance with ADHS and Joint Commission requirements and company policies
Build and maintain professional relationships with referring agencies
Qualifications
High School Diploma required; Bachelor’s or Master’s degree preferred
Minimum 1+ year of case management experience
At least 1 year of experience in substance abuse or behavioral health
Valid driver’s license and reliable transportation
AZDPS Level 1 Fingerprint Clearance Card
Current CPR/First Aid certification
Preferred: PRSS certification and behavioral health coursework

Physical Requirements
Ability to walk, bend, stand, and climb stairs
Lift up to 25 lbs occasionally
