Behavioral Health Consultant (LMFT, LPC, or LCSW)

Salary and Benefits:
- Salary: $74,866/year (negotiable based on experience)

Responsibilities:
- Provide brief, evidence-based behavioral interventions in a primary care setting
- Support patients through 30-minute sessions, in person or via telehealth
- Utilize a short-term model (2–5 visits) to address emotional and behavioral health concerns
- Screen and assess patients for mental health, substance use, and psychosocial needs
- Collaborate with primary care providers to integrate behavioral health into care plans
- Offer crisis intervention, care coordination, and referrals for long-term services when needed
- Educate patients on coping strategies, self-management, and behavioral change techniques
- Maintain concise and timely documentation in the EHR system
- Participate in team huddles, case conferences, and continuous quality improvement activities
Requirements:
- Active license in Pennsylvania as an LCSW, LPC, LMFT, or Psychologist
- Experience in integrated behavioral health or primary care settings preferred
- Skilled in brief, solution-focused, and evidence-based treatment modalities
- Strong communication and clinical documentation skills
- Ability to function effectively in a fast-paced, collaborative environment
- Passion for accessible, community-centered healthcare
